What's Wrong With Gremlin?

Diagnosis Can Be Made, And A Solution Found.
                                      Background Information On Gremlin
Gremlin will be 16 years old on December 7th, 2008.  His breed is a mixture of Toy Poodle and Shizu.
He was diagnosed with a heart murmur when he was 6 years old. About 4 years ago he developed an ongoing
cough that was suspected to be due to Congestive Heart Failure (CHF). At the present time he is on the following
medications for CHF: Enalapril (2.5mg), Furosemide (12.5mg), Pimobendan (1.25mg).
Over the last two years the cough has become increasingly worse. Lateral x-rays indicate that Gremlin has no
fluid in the lungs, and no fluid around the cardiac sac. He does have an enlarged heart, but this condition existed
years before the coughing started.   It is suspected that Gremlin's cough may actually be due to Collapsed
Trachea. X-rays indicate that the trachea is normal, but a fluoroscope  has never been done to see if the trachea
collapses during respiration.
Gremlin has a persistent cough that takes place when he turns his head in certain directions, or contorts his
body. It seems that these mechanical contortions irritate something in his airway that causes him to gag, and
cough.  Gremlin is extremely active for his age. He goes on at least 2, one mile walks per day.  He does not have
arthritis. He never coughs when he is walking. If he coughs uncontrollably during the day, going for a walk will
slowly cause the cough to dissipate. Also, Gremlin never coughs when he is asleep.  As soon as he wakes up the
coughing begins. This cough seems to have slowed Gremlin down slightly which is expected of any cough that
goes on for hours everyday.  On June 27, 2008 Gremlin was prescribed Lomotil.  This medication had no effect.
Next he was prescribed Butorphanol which made him sleepy, lose his appetite,  but did nothing for his cough.
Despite his cough Gremlin's appetite remains good, his weight remains the same, his walking schedule remains
the same, but his persistent cough is laboring, and is slowing him down a little everyday.
We need answers.
